Why should you get a Heart Health Check?
 
Heart disease is the leading cause of death in Australia. Many heart attacks and strokes can be prevented through
healthy behaviours like:
 
Eating a heart-healthy diet, being active, maintaining a healthy weight and being smoke free.
 
Many people may not know their risk of developing heart disease, especially since you often can’t feel risk factors like high blood pressure and cholesterol.
 
Having a Heart Health Check is an important first step to protecting your heart.
 
Who needs a Heart Health Check?
 
• All people aged 45–79 years
• People with diabetes from 35 years
• First Nations people from 30 years
 
Speak to your GP about a Heart Health Check if you do not already have heart disease.
 
How much does a Heart Health Check cost?
 
Heart Health Checks are covered by Medicare and bulk bill with our practice if you have a Medicare card.

MyMedicare – Voluntary Patient Registration

MyMedicare is a new voluntary patient registration model. It aims to strengthen the relationship between patients, their general practice, general practitioner (GP) and primary care teams.

Registration in MyMedicare is voluntary for patients, practices and providers and commenced on October 1st 2023.

 

Why is Important?

MyMedicare patients will have access to:

  • Greater continuity of care with their registered practice, improving health outcomes

  • Longer MBS-funded telephone calls (Levels C and D) with their usual general practice

  • Triple bulk billing incentive for longer MBS telehealth consultations (Levels C, D and E) for children under 16, pensioners, and concession card holders.

  • Chronic Disease Management items will be linked to a patient’s registration in MyMedicare from November 2024, to support continuity of care for people with chronic and complex conditions.

 

Step one:

Our practice has set up a consent form through Hotdoc for you to fill in. You will receive access to this form when you next make an appointment or by email. If you have already signed this, please go to step two. If not click on the link and fill in the form.

Step two:

There are two options for registering with a practice.

Option 1 – Register using your Medicare Online Account (MOA)

 

To register for MyMedicare using your Medicare Online Account, you’ll need:

  • to link your Medicare online account to myGov

  • your Medicare card.

To register online, follow the steps below:

  1. Sign in to myGov.

  2. Select Medicare.

  3. Select Register for MyMedicare, then Start.

  4. Under search for your practice, select Your practice, then Next. If you can’t find your practice, you’ll need to contact them to check if they’re registered for MyMedicare.

  5. Select Your preferred GP, then Next.

  6. Select Submit registration, then follow the prompts to complete your claim.

 

Option 2 – Register using the Express Plus Medicare mobile app

To register for MyMedicare using the Express Plus Medicare mobile app, you’ll need:

  • to link your Medicare online account to myGov

  • download the most up-to-date version of Express Plus Medicare mobile app

  • your Medicare card.

To register using the app, follow the steps below:

  1. Log into the Medicare Express Medicare mobile app.

  2. Select Register for MyMedicare, then Start.

  3. Under search for your practice, select Your practice, then Next. If you can’t find your practice, you’ll need to contact them to check if they’re registered for MyMedicare.

  4. Select Your preferred GP, then Next.

  5. Select Submit registration, then follow the prompts to complete your

WHAT IS NEW?

MyMedicare

The Australian Government is introducing a new voluntary patient registration model called MyMedicare to drive improvements in primary health care for all Australians and deliver new funding packages to primary care providers.


MyMedicare will strengthen the relationship between patients and their primary care teams by allowing patients to register with their regular general practice and select their preferred GP and providers within the practice.

Registration in MyMedicare is voluntary for patients, practices and providers.
Practices can now begin the MyMedicare registration process and link their providers to their practice before patient registration becomes available from 1 October 2023.

Why it is important?

 

 

MyMedicare patients will have access to:

 

  • Greater continuity of care with their registered practice, improving health outcomes

  • Longer MBS-funded telephone calls (Levels C and D) with their usual general practice

  • Triple bulk billing incentive for longer MBS telehealth consultations (Levels C, D and E) for children under 16, pensioners, and concession card holders

  • If they live in a residential aged care home, more regular visits from their GP and better care planning, from August 2024

  • If they visit hospital frequently, connections to more appropriate care in general practice, from the 2024-25 financial year.

Telehealth

Practices that provide services to patients who would benefit from the new MyMedicare-linked MBS longer telehealth services or provide care to people in residential aged care are encouraged to register those patients in MyMedicare as priority cohorts over the next 3 months.

Patients

·         Registration

To register in MyMedicare, patients must have either:

  • A Medicare card

  • Department of Veterans’ Affairs (DVA) Veteran Card.

Most patients will need to have 2 face-to-face visits recorded with the same practice in the previous 24 months to be eligible to register with that practice.

For patients registered with a practice in a remote location (in Modified Monash Model (MMM) 6 and 7 locations), this requirement is reduced to one face-to-face visit recorded at the same practice in the previous 24 months.

Parents/guardians and children can be registered at the same practice if one of them is eligible and registered at the practice.

 

Patients can only be registered with one practice at a time.

 

Eligible visits can be billed through:

  • Medicare

  • DVA

  • a combination of both.

Exemptions from all eligibility requirements will apply to people who are facing hardship including people experiencing domestic and family violence and/or homelessness. A trusted practice or GP will be able to help with direct registration in MyMedicare.
